Facility
The office is outfitted with only custom Precision Neurological Correction (PNC) equipment facilitating high quality, specific chiropractic care. No physical therapeutics, nor palliative procedures will be used as part of your in office care. You can be confident that you will feel right at home in this office as I strive to remove the impersonal barriers that we all experience too often in today's health care environment. The office is conveniently located near the Saratoga Avenue offramp of the 280 freeway at 4030 Moorpark Ave. #104, San Jose, CA.

Appointments
If you would like to make an appointment, please contact the office by phone or email. I will personally communicate with any new patient wishing to begin care in this office. Normal office hours are as follows:

Monday: 10 AM to 7 PM

Tuesday: By previous appointment only

Wednesday: 10 AM to 7:30 PM

Thursday: 10 AM to 3:00 PM

Friday: 10 AM to 7 PM

Saturday: Visits can be made on a special circumstances (i.e. emergency) basis and will be subject to a $50 cash surcharge above the normal fee schedule.

Financial
We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. With insurance benefits diminishing for chiropractic care in recent years, the great majority of my patients, realizing the value in their Precision Cervical Correction visits, pay for their services without 3rd party reimbursement. Forms of payment accepted by the office are personal check, cash, Visa or Mastercard.

We are nonparticipating Medicare providers meaning payment is due at the time of service we will bill Medicare for you. All forthcoming Medicare payments will then be mailed directly to the patient. Medicare does not cover the first visit (exam, history, any necessary x-rays). Medicare will only cover subsequent visits requiring a spinal correction that can be documented as being resultant to an acute functional change in the upper cervical spine.

We no longer accept third party personal injury lien cases. We will bill your automobile insurance policy Medpay for services or, if you have no Medpay, you can use your private health insurance or cash to cover your office fees.

Due to the nature of changes in the California workers compensation system, we no longer serve workers under this program in our office.

Time Requirements

First visit 60 minutes

Second visit 45 minutes

Subsequent visits 15-25 minutes

Cancellation
If you are unable to keep an appointment, we ask that you kindly provide at least a 24 hour notice.
